Introdução: O aumento da expectativa média de vida leva a população a buscar, com maior frequência, tratamentos estéticos que visem o rejuvenescimento da face e do pescoço. O Ultrassom microfocado (MFU) é descrito como uma opção de tratamento não-invasiva para a flacidez facial e o envelhecimento. Objetivo: Revisar a literatura científica quanto ao uso do MFU no processo de rejuvenescimento facial e correção da flacidez da pele. Metodologia: Trata-se de uma revisão narrativa da literatura, baseada na pesquisa de artigos científicos nas bases de dados PubMed e Biblioteca Virtual em Saúde realizada durante o mês de março de 2022, através da seguinte estratégia de busca: "Microfocused Ultrasound AND Facial Rejuvenation AND Skin Laxity Treatment". Um total de 19 manuscritos foram incluídos em síntese qualitativa. Desenvolvimento: O MFU pode ser aplicado no tecido subcutâneo, produzindo pequenos pontos de coagulação térmica dentro da camada reticular média a profunda da derme e subderme. A aplicação de calor nesses discretos pontos de coagulação térmica faz com que haja neossíntese de colágeno e consequente enrijecimento da pele. Estudos recentes evidenciaramresultados promissores quanto a aplicação do MFU em regiões de sulco nasolabial, linha da mandíbula, sobrancelhas, região infraorbital, palpebra inferior e ligamentos de retenção zigomático-bucais. Considerações finais: O MFU tem se mostrado uma técnica segura, sendo considerado eficaz, não-invasiva, bem tolerada pelos pacientes e viável para a prática clínica médica e odontológica.


Introduction: The increase in average life expectancy leads the population to seek, with greater frequency, aesthetic treatments aimed at rejuvenating the face and neck. Microfocused ultrasound (MFU) is described as a non-invasive treatment option for facial sagging and aging. Objective: To review the scientific literature regarding the use of MFU in the process of facial rejuvenation and correction of sagging skin. Methodology: This is a narrative review of the literature, based on the search for scientific articles in the PubMed and Virtual Health Library databases carried out during the month of March 2022, using the following search strategy: "Microfocused Ultrasound AND Facial Rejuvenation AND Skin Laxity Treatment". A total of 19 manuscripts were included in a qualitative synthesis. Development: MFU can be applied to the subcutaneous tissue, producing small points of thermal coagulation within the middle to deep reticular layer of the dermis and subdermis. The application of heat to these discrete points of thermal coagulation causes collagen neosynthesis and consequent skin tightening. Recent studies have shown promising results regarding the application of MFU in regions of the nasolabial sulcus, jaw line, eyebrows, infraorbital region, lower eyelid and zygomatic-buccal retention ligaments. Final considerations: MFU has been shown to be a safe technique, being considered effective, non-invasive, well tolerated by patients and viable for clinical medical and dental practice.

Purpose: Low-intensity pulsed ultrasound (LIPUS) has been used to stimulate the healing of the fresh fracture, delayed union, and non-union in both animal and clinical studies. Besides, biphasic calcium phosphate ceramic (BCP) is a promising biomaterial for bone repair as it shows favorable biocompatibility, osteoinduction, and osteoconduction. However, scarcity is known about the combined effect of LIPUS and BCP on bone formation. Methods: The combined effect of LIPUS and BCP was studied in a beagle model. Twelve dogs were used. BCP granules without any additions were implanted into bilateral erector spinae muscles. One side is the BCP group, while the counterlateral side is LIPUS + BCP group. Histological and histomorphometric analyses, and quantitative real-time polymerase chain reaction were evaluated. Results: Compared with BCP alone, the LIPUS + BCP showed no advantages in early bone formation. Furthermore, the Notch signaling pathway-related mRNA has no significant difference between the two groups. Conclusions: The preliminary results showed that the BCP, which has intrinsic osteoinduction nature, was an effective and promising material. However, LIPUS has no enhanced effect in BCP induced ectopic bone formation. Furthermore, LIPUS has no effect on the Notch signaling pathway. Whether costly LIPUS could be used in combination with BCP should be a rethink.

Abstract Background: The present study investigated the effects of pulsed and continuous ultrasound (USP and USC) in edema and hyperalgesia after chronic inflammatory process induced by Complete Freund's Adjuvant-CFA and analyzing the relationship of the application frequency of ultrasound, in pro- and anti-inflammatory cytokine production. Methods: Forty-five animals were divided into 9 groups; all animals from groups 2 to 9 were subjected to a persistent inflammation model induced by CFA in mice. We report the effects and the underlying action mechanisms of USP and USC in the animals which were irradiated two, three or five times a week on the left hind paw. The analyses performed in this study were: evaluation of hind paw edema through the plethysmometer, evaluation of thermal hyperalgesia through withdrawal test using a water container at 44.5°C (± 0.5°C), and the plantar region of the left paw which was removed for analysis of cytokines. Results: Our results showed that USP and USC consistently reduced paw edema, and pulsed ultrasound showed a higher significant effect than the continuous mode. Moreover, groups with irradiation frequency of five times a week presented an inhibition of the edema, and groups with frequency of three or two times a week reduced mainly hyperalgesia, in comparison with the control group. The beneficial effects of the US then seem to be associated with upregulation of anti- and pro-inflammatory mediators, such as IL-10 and IL-6, respectively. Conclusion: This study provided evidence that ultrasound constitutes an important non-pharmacological intervention for the management of inflammatory and pain states.

Introduction : l'échographie est l'examen de première intention dans l'exploration morphologique de la thyroïde. Elle constitue un excellent outil diagnostique et pronostique dans les thyropathies nodulaires. Objectif : déterminer le profil des nodules thyroïdiens à l'échographie selon la classification EU-TIRADS. Matériel et méthodes : étude transversale descriptive réalisée dans le service de radiologie du CHU de Bouaké durant une année. Nous avons inclus tous les comptes rendus d'échographie thyroïdienne des patients ayant des nodules thyroïdiens. Les données ont été analysées à l'aide du logiciel Epi Info 7. Résultats : Au total, 60 patients présentaient des nodules sur 97 reçus pour thyropathies soit 60,8%. L'âge moyen était de 45,6 ans [13-82 ans]. Legenre féminin prédominait avec 82,7%. La tuméfaction cervicale antérieure était la principale indication dans 72%. Le volume thyroïdien était augmenté chez 63%des patients avec un volume moyen de 71 cm3. Il s'agissait de nodule solitaire dans 46,55%. Les nodules spongiformes prédominaient dans 32%. Les nodules étaient solides dans 29,8% et mixtes dans 16,5.%. L'hypo-échogénicité était retrouvée dans 44,8% des cas de nodules solides. Les lésions étaient classées EU-TIRADS 2 dans 58,8%, EU-TIRADS 3 dans 23,5%, EU-TIRADS 4 dans 14,7% et EU-TIRADS 5 dans 2,9%. Conclusion : la caractérisation et la classification des nodules thyroïdiens constituent une étape importante pour la prise en charge des patients. Ces nodules dans notre étude sont essentiellement d'allure bénigne (EU-TIRADS 2 et 3) avec une prédominance féminine. Un arbre décisionnel tenant compte des éléments cliniques et cytopathologiques servirait davantage pour d'éventuelles chirurgies.


Introduction: Ultrasound is the first-line examination in the morphological exploration of the thyroid. It is an excellent diagnostic and prognostic tool in nodular thyropathy. Objective: to determine the profile of thyroid nodules on ultrasound according to the EU-TIRADS classification. Material and methods: descriptive cross-sectional study carried out in the radiology department of the University Hospital of Bouaké during one year. We included all thyroid ultrasound reports of patients with thyroid nodules. Data were analysed using Epi Info 7 software. Results: A total of 60 patients had nodules out of 97 referred for thyropathy (60.8%). The mean age was 45.6 years [13-82 years]. Females predominated with 82.7%. Anterior cervical swelling was the main indication in 72%. Thyroid volume was increased in 63% of patients with a mean volume of 71 cm3. Solitary nodules were present in 46.55%. Spongiform nodules were predominant in 32%. The nodules were solid in 29.8% and mixed in 16.5%. Hypoechogenicity was found in 44.8% of solid nodules. Lesions were classified as EU-TIRADS 2 in 58.8%, EU-TIRADS 3 in 23.5%, EU-TIRADS 4 in 14.7% and EU-TIRADS 5 in 2.9%. Conclusion: The characterisation and classification of thyroid nodules is an important step in the management of patients. In our study, these nodules are essentially benign (EU-TIRADS 2 and 3) with a female predominance. A decision tree taking into account the clinical and cytopathological elements would be more useful for possible surgery.

Objective: This study aimed to compare the effect of ibuprofen and low intensity pulsed ultrasound (LIPUS) on the reduction of pain after the placement of initial archwire in orthodontic patients. Material and Methods: This double-blind clinical trial study was carried out on 60 female candidates for fixed orthodontic treatment referring to the Orthodontic Department of School of Dentistry in Mashhad University of Medical Sciences, Mashhad, Iran, during 2015-2016. The subjects were divided into four groups of ibuprofen, LIPUS, placebo, and mock LIPUS. A questionnaire and a rectangular and flexible cubic silicone were given to each patient to record the severity of pain based on the visual analog scale at specified time points (i.e., 2 h, 6 h, at bedtime, 2nd, 3rd, and 7th days after archwire placement) when biting the silicone block with the anterior and posterior teeth and without biting at all. Repeated measures analysis of variance was used in order to compare the pain severity at different time points. Results: The comparison of pain severity at various time points showed that the highest and lowest mean scores of pain were reported at bedtime and seven days after the intervention (p<0.001). In each of the three conditions (i.e., biting the silicone block with the anterior and posterior teeth and without biting the teeth) at six time points (i.e., 2 h, 6 h, at bedtime, 2nd, 3rd, and 7th days following archwire placement), no significant difference was observed in the severity of pain (p>0.05). Conclusion: In conclusion, LIPUS (with a frequency of 1 MHz and an intensity of 100 mW) and ibuprofen have no significant effects on reduction of the pain severity at different time points and various conditions in orthodontic patients.


Objetivo: Este estudio tuvo como objetivo comparar el efecto del ibuprofeno y el ultrasonido pulsado de baja intensidad (LIPUS) en la reducción del dolor después de la colocación del arco inicial en pacientes de ortodoncia. Material y Métodos: Este estudio de ensayo clínico doble ciego se llevó a cabo en 60 candidatas a tratamiento de ortodoncia fija referidas al Departamento de Ortodoncia de la Facultad de Odontología de la Universidad de Ciencias Médicas de Mashhad, Mashhad, Irán, durante 2015-2016. Los sujetos se dividieron en cuatro grupos: ibuprofeno, LIPUS, placebo y LIPUS simulado. Se entregó un cuestionario y un bloque de silicona cúbica rectangular y flexible a cada paciente para registrar la intensidad del dolor según la escala analógica visual en puntos de tiempo específicos (es decir, 2 h, 6 h, hora de acostarse, 2do, 3er y 7mo día después de la colocación del arco) al morder el bloque de silicona con los dientes anteriores y posteriores, y sin morder en absoluto. Se utilizó el análisis de varianza de medidas repetidas para comparar la intensidad del dolor en diferentes momentos.Resultados: La comparación de la intensidad del dolor en varios puntos de tiempo mostró que las puntuaciones medias de dolor más altas y más bajas se informaron a la hora de acostarse y siete días después de la intervención (p<0,001). En cada una de las tres condiciones (es decir, al morder el bloque de silicona con los dientes anteriores y posteriores, y sin morder) en seis momentos (2 h, 6 h, antes de acostarse 2do, 3er y 7mo día después de la colocación del arco), no se observó diferencia significativa en la severidad del dolor (p>0.05).Conclusión: En conclusión, LIPUS (con una frecuencia de 1 MHz y una intensidad de 100 mW) y el ibuprofeno no tienen efectos significativos en la reducción de la severidad del dolor en diferentes puntos de tiempo y diversas condiciones en pacientes de ortodoncia.

INTRODUCTION: The skin flap is a surgical technique widely used in clinical practice and generally presents postoperative complications. Therefore, elucidating interventions that assist in tissue conservation is essential. Photobiomodulation (PBM) and therapeutic ultrasound (TUS) are non-invasive alternatives for assisting tissue repair, however, there is no consensus on the parameters used. OBJECTIVE: To describe the effectiveness of the different parameters of PBM and TUS in the viability of the dorsal random pattern skin flap in mice. METHODS: Fifty-five Swiss mice were used, distributed in eleven groups. The animals were submitted to surgical technique including revascularization of the area limited through a plastic barrier (polyester/polyethylene) with the same dimension as the flap. PBM or TUS was applied for five consecutive days. Photographic and thermographic recordings were performed with Cyber-Shot DSC-P72 and FlirC2 cameras and analyzed using the ImageJ® and FLIR Tools software, respectively. In the statistical analysis, the data were submitted to the GraphPad Prism® 8.0 software. Analysis of variance (ANOVA Two-way) and Tukey's post-test was performed, considering 5% significance level. RESULTS: Groups 5 (PBM830 nm; 10 J/cm²) and 6 (TUS 3 MHz; 0.4 W/cm²) showed percentages of viable tissue significantly higher on the third and fifth day of the experiment, when compared to the other groups. The temperature decreased significantly in group 1 when compared to the others in the postoperative period. CONCLUSION: The continuous TUS at 3 MHz and PBM 830 nm were more effective in improving the viability of the dorsal random pattern skin flap in mice.


INTRODUÇÃO: O retalho cutâneo é uma técnica cirúrgica amplamente empregada na prática clínica e comumente apresenta complicações pós-operatórias. Portanto, elucidar intervenções que auxiliem na conservação do tecido são fundamentais. A fotobiomodulação (FBM) e o ultrassom terapêutico (UST) são alternativas não invasivas que auxiliam no reparo tecidual, contudo, ainda não há consenso sobre os parâmetros a serem utilizados. OBJETIVO: Descrever a efetividade dos diferentes parâmetros da FBM e do UST na viabilidade do retalho cutâneo randômico dorsal em camundongos. MÉTODOS: Utilizou-se 55 camundongos Swiss, distribuídos em onze grupos. Os animais foram submetidos à técnica cirúrgica com a revascularização da área limitada através de uma barreira plástica (poliéster/polietileno) da mesma dimensão do retalho. Aplicou-se a FBM ou UST durante cinco dias consecutivos. O registro fotográfico e termográfico foi realizado com as câmeras Cyber-Shot DSC-P72 e FlirC2, sendo posteriormente analisados nos softwares ImageJ® e FLIR Tools, respectivamente. Na análise estatística, os dados foram submetidos ao software GraphPad Prism® 8.0 e ao teste Shapiro-Wilk para a análise da normalidade. Realizou-se a análise de variância (ANOVA Two-way) e pós-teste de Tukey, com nível de significância de 5%. RESULTADOS: Os grupos 5 (FBM 830 nm; 10 J/cm²) e 6 (UST 3 MHz; 0,4W/cm²) apresentaram porcentagens de tecido viável significativamente maiores no terceiro e quinto dia do experimento. A temperatura reduziu significativamente no grupo-1 quando comparado aos demais no pós-operatório. CONCLUSÃO: O UST contínuo a 3 MHz e FBM 830 nm, foram mais eficazes melhorando a viabilidade a do retalho cutâneo randômico dorsal em camundongos.

Résumé

O objetivo deste estudo foi avaliar a efetividade antimicrobiana e a dor pós-operatória (PP) da irrigação ultrassônica (IU) em comparação com a irrigação convencional (CI), por meio de duas revisões sistemática e meta-análises de ensaios clínicos randomizados, para isto foram produzidos dois artigos, um para dor pós operatória e outro para avaliação antimicrobiana, desta forma a dissertação a seguir contará com dois capítulos. Essa revisão foi elaborada seguindo o guia PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses). Após elaborada a pergunta clínica e a estratégia PICO de cada estudo, uma pesquisa bibliográfica foi realizada nas principais bases de dados científicas por meio de uma estratégia de busca elaborada com termos MeSH e termos livres adaptados para as bases de dados. As meta-análise foram conduzidas usando o software R com o pacote "META", o efeito de medida de diferença média (MD) e odds ratio (OR) foi calculada e o modelo de efeito fixo foi aplicado com um intervalo de confiança (IC) de 95%. A escala da colaboração Cochrane foi usada para avaliar o risco de viés e a ferramenta GRADE para avaliar a qualidade das evidências. Os resultados mostraram vantagem favorecendo o grupo irrigação ultrassônica em ambas as variáveis de interesse (dor pós-operatória e efetividade antimicrobiana), na dor pós-operatória, 6 ensaios clínicos randomizados (RCTs) foram incluídos para revisão sistemática e quatro para meta-análise. IU resultou em menor PP em 3 dos 5 períodos, 6 horas (MD -1,40 [CI -2,38 a - 0,42] p = 0,0052), 24 horas (MD -0,73 [CI -1,07 a -0,39] p = 0,0001), e 48 horas (MD -0,36 [CI -0,59 a -0,13] p = 0,022). No entanto, a PP não apresentou diferenças significativas entre os grupos em 72 horas e 7 dias (p> 0,05). Um baixo risco de viés foi observado para a maioria dos domínios, exceto a alocação que foi considerada pouco clara. A certeza da evidência foi classificada em moderada (24 horas, 48 horas e 7 dias) e baixa (6 e 72 horas). Já na efetividade antimicrobiana, 12 RCTs foram incluídos para a revisão sistemática e oito para as meta-análises onde 4 foram utilizadas para (MD) e 4 para (OR). Em ambas análises a IU resultou em melhor efeito antimicrobiano em comparação com a CI MD -1,42 [-1,60; -1,23] p < 0,0001, I2 = 80% e OR 3.86 [1.98; 7.53] p< 0.0001, I2 = 28.7%. Um baixo risco de viés foi observado para a maioria dos domínios, exceto a alocação que foi considerada pouco clara. A certeza das evidências foi considerada moderada na meta-análise utilizando OR, devido aos achados de imprecisão, e baixa na meta-análise utilizando MD devido a presença de inconsistência e imprecisão. Desta forma é possível concluir que dentro das limitações das presentes revisões sistemáticas a IU apresentou resultados favoráveis tanto para dor pós-operatória quanto para o aumento da efetividade antimicrobiana. Contudo ensaios clínicos randomizados mais robustos são necessários para corroborar com esses achados(AU)


The aim of this study was to evaluate the antimicrobial effectiveness and postoperative pain (PP) of ultrasonic irrigation (UI) compared to conventional irrigation (CI), through two systematic reviews and meta-analyses of randomized clinical trials for this, two articles were produced, one for postoperative pain and another for antimicrobial evaluation, so the dissertation below will have two chapters. This review was prepared following the PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses). After preparing the clinical question and the PICO strategy for each study, a literature search was carried out in the main eletronic databases through a search strategy elaborated with MeSH terms and free terms adapted to the databases. Meta-analyses were conducted using the R software with the "META" package, the mean difference (MD) and odds ratios (OR) was the measure effect necessary and the fixed-effect model was applied with a 95% confidence interval (CI). The Cochrane Collaboration Scale was used to assess the risk of bias and the GRADE tool to assess the quality of evidence. The results showed an advantage favoring the ultrasonic irrigation group in both variables of interest (postoperative pain and antimicrobial effectiveness), in postoperative pain, 6 RCTs were included for systematic review and four for metaanalysis. UI resulted in lower PP in 3 of the 5 periods, 6 hours (MD -1.40 [CI -2.38 to -0.42] p = 0.0052), 24 hours (MD -0.73 [CI -1 .07 to -0.39] p = 0.0001), and 48 hours (MD -0.36 [CI -0.59 to -0.13] p = 0.022). However, PP did not show significant differences between groups at 72 hours and 7 days (p> 0.05). A low risk of bias was observed for most domains, except the allocation was considered unclear. The certainty of the evidence was classified as moderate (24 hours, 48 hours and 7 days) and low (6 and 72 hours). In the study of antimicrobial effectiveness, 12 RCTs were included for the systematic review and 8 for the meta-analyses where 4 were used for (MD) and 4 for (OR). In both analyses, UI resulted in better antimicrobial effect compared to CI MD -1.42 [- 1.60; -1.23] p< 0.0001, I2 = 80% and OR 3.86 [1.98; 7.53] p< 0.0001, I2 = 28.7%. A low risk of bias was observed for most domains, except the allocation was considered unclear. The certainty of evidence was considered moderate in the meta-analysis using OR, due to the imprecision findings, and low in the meta-analysis using MD due to the presence of inconsistency and imprecision. Thus, it is possible to conclude that, within the limitations of the present systematic reviews, UI presented favorable results both for postoperative pain and for the increase in antimicrobial effectiveness. However, more robust randomized controlled trials are needed to corroborate these findings(AU)

ABSTRACT Objective: To evaluate the efficacy of low-cost nucleus fragmenting forceps designed to reduce the use of ultrasound during phacoemulsification. Methods: A total of 60 pig eyes enucleated 10 hours before were placed in the microwave oven, at maximum power for 10 seconds, to form cataracts with hardness comparable to a grade IV nucleus in the Lens Opacities Classification System III. Cataract extraction was performed using the Centurion® phacoemulsifier (Alcon Laboratories, Geneve, Switzerland) and Leica M620 microscope. All eyes were submitted to the pre-chop technique dividing the nucleus into four parts. After the pre-chop, the quadrants in 30 eyes were phacoemulsified with the torsional mode and were fragmented in the remaining 30 eyes after the pre-chop was with the fragmentation forceps before torsional mode phacoemulsification. The device was calibrated for all eyes by applying the following parameters: 40% linear torsional phacoemulsification; intraocular pressure of 65 mmHg; the linear vacuum of 600 mmHg; aspiration flow of 40 ccs/minute. After each procedure, the following was recorded: cumulative dissipated energy; equivalent average torsional amplitude; equivalent average ultrasonic power; estimated aspirated fluid; ultrasound total time; and total aspiration time. Statistical analysis was performed using the Kruskal-Wallis test and the IBM Statistical Package for Social Sciences. The p-value <0.05 was considered statistically significant. Results: There was a statistically significant reduction favoring the use of the nucleus fragmenting forceps in all parameters, except for the average torsional amplitude. Conclusion: The use of the nucleus fragmenting forceps contributed to improving the efficacy of torsional phacoemulsification in enucleated pig eyes.


RESUMO Objetivo: Avaliar a eficácia de uma pinça fragmentadora de núcleo, de baixo custo, desenvolvida para reduzir o uso de ultrassom durante a emulsificação do núcleo. Métodos: Sessenta olhos de porco com 10 horas de enucleação foram colocados no forno de microondas, em potência máxima por 10 segundos, para a formação de catarata com dureza comparável à de um núcleo grau IV na Lens Opacities Classification System III. A extração da catarata foi realizada com o facoemulsificador Centurion® (Alcon Laboratories, Genebra, Suíça) e microscópio Leica M620. Todos os olhos foram submetidos a técnica de pre-chop, dividindo o núcleo em quatro partes. Em 30 olhos, após o pre-chop, foi feita a facoemulsificação dos quadrantes com o modo torsional e, nos outros 30 olhos, após o pre-chop, cada quadrante foi fragmentado com a pinça antes da facoemulsificação com o modo torsional. O aparelho foi calibrado para todos os olhos com os seguintes parâmetros: faco torsional linear 40%; pressão intraocular 65 mmHg; vácuo linear 600mmHg e fluxo de aspiração 40cc/minuto. Após cada procedimento, verificaram-se energia dissipada acumulada; média da amplitude do faco torsional; média equivalente do poder ultrassônico; líquido aspirado estimado; tempo total de ultrassom e tempo total de aspiração. A análise estatística foi realizada utilizando o teste de Kruskal-Wallis com o IBM Statistical Package for Social Sciences. Valor de p<0,05 foi considerado estatisticamente significante. Resultados: Houve redução estatisticamente significante em favor do uso da pinça fragmentadora de núcleo em todos os parâmetros, menos na média de amplitude do faco torsional. Conclusão: O uso da pinça fragmentadora de núcleo contribuiu para melhorar a eficácia do faco torsional em olhos de porco enucleados.

O tratamento periodontal consiste na remoção do biofilme patogênico, através da raspagem e alisamento radicular. O desbridamento ultrassônico de boca toda promove uma instrumentação mais conservadora, porém eficiente da superfície radicular, em sessão única. Evitando a translocação bacteriana de uma região tratada para outra que já foi. O objetivo do presente trabalho foi realizar uma comparação entre a eficácia da raspagem manual e a ultrassônica dentro do protocolo da FMD, através de um relato de caso clínico. Houve uma melhora nos parâmetros clínicos periodontais em todos os quadrantes, porém resultados superiores foram observados com o desbridamento com ultrassom e irrigação com clorexidina. A instrumentação com ultrassom associada a clorexidina no tratamento da periodontite estágio III grau C generalizada, reduz com eficácia o tempo de tratamento, otimizando o tempo do paciente e profissional(AU)


Periodontal treatment consists of removing the pathogenic biofilm, by scaling and root planing. Ultrasonic debridement of the entire mouth promotes more conservative, yet efficient instrumentation of the root surface, in a single session. The objective of the present study was to make a comparison between the effectiveness of manual and ultrasonic scraping within the FMD protocol, through a clinical case report. There was an improvement in periodontal clinical parameters in all quadrants, but superior results were observed with debridement with ultrasound and irrigation with chlorhexidine. Low-intensity pulsed ultrasound (LIPUS) is a promising therapy that has been increasingly explored in basic research and clinical applications. LIPUS is an appealing therapeutic option as it is a noninvasive treatment that has many advantages, including no risk of infection or tissue damage and no known adverse reactions. LIPUS has been shown to have many benefits including promotion of tissue healing, angiogenesis, and tissue regeneration; inhibition of inflammation and pain relief; and stimulation of cell proliferation and differentiation. The biophysical mechanisms of LIPUS remain unclear and the studies are ongoing. In recent years, more and more research has focused on the relationship between LIPUS and stem/progenitor cells. A comprehensive search of the PubMed and Embase databases to July 2020 was performed. LIPUS has many effects on stem cells. Studies show that LIPUS can stimulate stem cells in vitro; promote stem cell proliferation, differentiation, and migration; maintain stem cell activity; alleviate the problems of insufficient seed cell source, differentiation, and maturation; and circumvent the low efficiency of stem cell transplantation. The mechanisms involved in the effects of LIPUS are not fully understood, but the effects demonstrated in studies thus far have been favorable. Much additional research is needed before LIPUS can progress from basic science research to large-scale clinical dissemination and application.

Objetivo: Comparar la eficacia de la inmovilización nocturna de la muñeca con una férula cubital en ángulo neutro junto con la aplicación de ultrasonido en pacientes con síndrome del túnel carpiano leve y moderado. Materiales y Métodos: Entre octubre de 2007 y marzo de 2010, se incluyó a pacientes >18 años con síndrome del túnel carpiano confirmado por electromiografía en un hospital de Buenos Aires. Se realizó una aleatorización estratificada, con bloques permutados aleatorios, y apareamiento por sexo y edad. Los pacientes fueron asignados al grupo experimental (GE) o al grupo de control (GC). Ambos grupos recibieron ultrasonido de 1 MHz pulsante por 15 min, 3 veces por semana, durante 6 semanas. Los pacientes del GE, además, utilizaron una férula nocturna. Se evaluaron el dolor y la parestesia con la escala analógica visual de 100 mm, la PSFS y el test de Moberg, al comenzar, a las 3 semanas y, al finalizar, a las 6 semanas, y durante el seguimiento, al mes, y a los 3 y 6 meses, con evaluador a ciego. Resultados: Se analizó a 32 pacientes del GC y a 33 del GE. Al finalizar el tratamiento, todas las variables habían mejorado en ambos grupos, con diferencia de medias estadísticamente significativa para el dolor a favor del GE a las 3 semanas de tratamiento 1,64 (IC95% 0,38-2,91; p = 0,012), pero sin diferencia clínica significativa. No se informaron efectos adversos. Conclusión: El tratamiento con una férula nocturna y ultrasonido no es superior al ultrasonido solo en pacientes con STC. Nivel de Evidencia; II


Objective: To compare the effectiveness of night wrist immobilization using an ulnar splint in neutral angle versus the use of ultrasound (US) in patients with Carpal Tunnel Syndrome (CTS). Materials and Methods: Study population included over 18 years of age that were treated for electromyography-confirmed CTS between October 2007 and March 2010 at a Buenos Aires hospital. A sex- and age-stratified randomization was performed by using randomly permuted blocks, allocating patients into the experimental group (EG) and control group (CG). Pulsed US therapy was administered for 15 minutes to all patients three times a week for six weeks at a frequency of 1 MHz. In addition, EG patients were also prescribed night splint. Pain and paresthesia were evaluated using a 100mm Visual Analogue Scale (VAS), the Patient Specific Functional Scale (PSFS), and the Moberg pickup test (MPUT) at baseline, at 3 and 6 weeks, and at 3 and 6 months after treatment institution by a blinded investigator. Results: Study population consisted of 85 cases (65 patients) that were randomly allocated to CG (n=42) or EG (n=43). Improvement of all the variables was observed at the end of treatment in both groups, with a 1.64 (95% CI: 0.38-2.91, P=0.012) statistically significant difference in means for pain in favor of the EG at 3 weeks of treatment, but without a significant clinical difference. No adverse effects were observed. Conclusion: The effectiveness of combined night splint and US therapy is not superior to the US alone treatment in CTS patients. Level of Evidence; II

SUMMARY: The aim of this study was to evaluate the effects of stretching and therapeutic ultrasound (TUS) on desmin and laminin contents of rat muscle after contusion. Male Wistar rats (n = 35, 8-9 weeks of age, 271 ± 14g body weight) were divided into five groups: Control group (CG) (n= 03); Injured group (IG) (n= 8); Injured + ultrasound group (IUSG) (n= 8); Injured+stretching group (ISG) (n= 8); Injured +ultrasound + stretching group (IUSSG) (n= 8). The application of ultrasound started 72 hours after the contusion, using the 50 % pulsed mode, 0.5 W/cm2, 5 min, once a day, for five consecutive days. Passive manual stretching was started on the tenth day after injury, with four repetitions of 30 s each and 30 s rest between repetitions, once a day, five times per week, for a total of ten applications. After 22 days, the rats were euthanazied and the gastrocnemius of both limbs removed for desmin and laminin immunohistochemistry morphometric measurement. Analysis was conducted using ANOVA one way post-hoc Tukey to parametric data and Kruskall-Wallis for non-parametric data. The IUSSG animals showed a larger area of desmin than ISG (p<0.05). It was found a decrease in laminin comparing IUSG to IG. However, laminin area was higher in ISG than all groups (p<0.05). UST isolated or in combination with stretching influenced gastrocnemius regeneration in different manners. While stretching applied isolated enhanced gastrocnemius regeneration noticed by the increase in laminin area, in combination with TUS strengthened the muscle healing rising desmin area.


RESUMEN: El objetivo de este estudio fue evaluar los efectos del estiramiento y la ecografía en los contenidos de desmina y laminina del músculo de rata después de la lesión. Ratas Wistar macho (n = 35, 8-9 semanas de edad, 271 ± 14 g de peso corporal) se dividieron en cinco grupos: grupo de control (CG) (n = 03); Grupo lesionado (GL) (n = 8); Lesionado + grupo de ultrasonido (LGU) (n= 8); Lesionado + grupo de estiramiento (LGE) (n = 8); Lesionado + ultrasonido + grupo de estiramiento (LUGE) (n = 8). La aplicación de ultrasonido comenzó 72 horas después de la lesión, usando el modo pulsado al 50 %, 0,5W / cm2, 5 min, una vez al día, durante cinco días consecutivos. El estiramiento manual pasivo se inició el décimo día después de la lesión, con cuatro repeticiones de 30 seg cada una y 30 seg de descanso entre repeticiones, una vez al día, cinco veces por semana, para un total de diez aplicaciones. Las ratas fueron sacrificadas después de 22 días, y se extrajo el músculo gastrocnemio de ambos miembros para la medición morfométrica de desmina y laminina a través de inmunohistoquímica. El análisis se realizó utilizando ANOVA unidireccional Tukey post-hoc para datos paramétricos y Kruskall-Wallis para datos no paramétricos. Los animales LUGE mostraron un área mayor de desmina que LGE (p <0,05). Se encontró una disminución en la laminina comparando LGU con GL. Sin embargo, el área de laminina fue mayor en LGE que en todos los grupos (p <0,05). El tratamiento con ultrasonido aislado o en combinación con estiramiento influyó en la regeneración del músculo gastrocnemio de diferentes maneras. Si bien el estiramiento aplicado, en combinación con tratamiento de ultrasonido, fortaleció el área de desmina, la regeneración del músculo gastrocnemio mejoró por el aumento en el área de laminina aumentando la curación muscular.

Objetivo: revisar a literatura quanto ao uso de soluções no reservatório líquido do ultrassom (US) para o tratamento das doenças periodontais, avaliando as contribuições dessa associação e as soluções mais eficazes para essa terapêutica. Métodos: foi realizada pesquisa bibliográfica nas bases de dados Public Medline (PubMed), Lilacs, Science Direct e ISI web of knowledge. As palavras-chave utilizadas foram: "periodontal disease", "ultrasonic", "cooling solution", "antimicrobial" e "irrigation". Como critérios de inclusão, foram selecionados estudos prospectivos, retrospectivos, caso controle, transversal e relatos de casos, publicados em inglês. Resultados: após pesquisa e minuciosa revisão por título e resumo de cada estudo, 15 estudos foram selecionados para avaliação dos desfechos investigados. As principais soluções associadas ao US foram povidono-iodo (PVPI), clorexidina, óleos essenciais, bicarbonato de sódio. O irrigante mais utilizado foi o PVPI, seguido por clorexidina e óleos essenciais. Os agentes podem conferir discretas melhoras no nível clínico de inserção e profundidade de sondagem, porém os resultados são controversos, já que nem todos os ensaios demonstraram efeitos positivos. Uma importante evidência observada foi em relação à redução dos níveis de microrganismos derivados do aerossol do US quando essas substâncias foram utilizadas no reservatório de água, sendo esse um aspecto positivo de seu uso como solução refrigerante ao US. Considerações finais: de modo geral, não puderam ser confirmados benefícios adicionais dos irrigantes antimicrobianos nos principais parâmetros periodontais avaliados, porém não se pode descartar o potencial uso desses agentes para a redução da dispersão de microrganismos advindos do aerossol, promovendo assim maior proteção ao paciente e ao profissional.(AU)


Objective: to review the literature regarding the use of solutions in the liquid ultrasonic (US) reservoir devices for the treatment of periodontal diseases, evaluating the benefits of this association, and the most effective solution for this therapy. Methods: bibliographic research was carried out in the Public Medline (PubMed), Lilacs, Science Direct, and ISI web of knowledge databases. The keywords used were: "periodontal disease", "ultrasonic", "cooling solution", "antimicrobial", and "irrigation". Inclusion criteria were prospective, retrospective, case-control, cross-sectional studies, and case reports published in English. Results: after a meticulous analysis of each paper by title and summary, 15 studies were selected for further investigation of clinical outcomes. The main solutions associated with US devices as coolant were PVPI, Chlorhexidine, Essential Oils and Sodium Bicarbonate. The most used coolant agent was PVPI, followed by chlorhexidine and essential oils. It has been shown that irrigating solutions can provide improvements in the clinical attachment level and probing depth, but the results are controversial since not all trials showed positive effects. Important evidence observed was the reduction of the levels of microorganisms derived from the US aerosol when these substances were used as a cooling solution. Conclusions: in general, additional benefits of coolant solutions could not be confirmed in the main periodontal parameters evaluated, however, the potential use of these agents to reduce the dispersion of microorganisms derived from the aerosol cannot be ruled out, thus promoting additional protection to the patient and professional.(AU)

Introdução: Acidentes e complicações podem acontecer em qualquer fase do tratamento endodôntico. Podem ocorrer devido a diversos fatores, como falhas dos instrumentos, dificuldades anatômicas e limitações do profissional, o que, muitas vezes, pode alterar o prognóstico de um caso. Objetivo: O presente relato de caso aborda uma situação em que uma paciente se apresentava com dois instrumentos fraturados e uma perfuração radicular em um dente molar inferior com periodontite apical. Relato do caso: Os fragmentos foram removidos por meio de técnica mecanizada e ultrassônica padronizada, e a perfuração foi tratada e selada com material à base de silicato de cálcio. Conclusão: O emprego de recursos tecnológicos atuais favoreceu a resolução do caso, melhorando seu prognóstico. O restabelecimento da condição de normalidade do dente e tecidos adjacentes foi confirmado por exames radiográficos e tomográficos (AU).


Introduction: Accidents and complications may happen at any endodontic treatment stage. These are likely to change the prognosis of treatment and can be due to factors related to instrumental failure, anatomic difficulties and the professional's limitations. Objective: This case report addresses a case where a patient presenting two separated instruments and a root perforation in a lower molar with apical periodontitis. Case report: The separated instruments have been removed by mechanical and ultrasonic standardized method and the perforation was treated and sealed by silicate cement-based material. Conclusion: The use of technological resources favored the resolution of the case, increasing its predictability. The reestablishment of the normal condition of the tooth and surrounding tissues was confirmed by radiograph and computed tomography exams (AU).

INTRODUCCIÓN: El tromboembolismo pulmonar (TEP) es una causa frecuente de morbimortalidad cardiovascular y la trombolisis local asistida por ultrasonido (TLUS) constituye una alternativa de tratamiento validada para pacientes de riesgo intermedio. No existen reportes del uso de esta tecnología en el ámbito nacional. MÉTODOS: Análisis descriptivo, prospectivo, sobre una serie de pacientes con TEP agudo, de riesgo intermedio, tratados en forma percutánea con trombolisis local asistida por catéter de ultrasonido terapéutico (EKOSonic®). Se analiza la eficacia y seguridad del procedimiento mediante variables clínicas, hemodinámicas y radiológicas, así como desenlaces intra hospitalarios. Se reporta, además, el seguimiento a 30 días. RESULTADOS: Entre Junio de 2019 y Marzo de 2020, 4 pacientes con TEP de riesgo intermedio fueron tratados con esta técnica. El score PESI (Pulmonary Embolism severity Index) promedio era de 79,5 + 6,1. Dos pacientes requirieron la instalación de 2 catéteres para efectuar terapia bilateral. Se observó una reducción promedio en la presión sistólica de arteria pulmonar de 29% y en 3 de los 4 enfermos se logró revertir la dilatación ventricular derecha presente al ingreso. La carga trombótica se redujo en un 20% medido por score CTOI. No hubo complicaciones intraoperatorias ni intrahospitalarias asociadas a la intervención. CONCLUSIONES: En esta serie inicial, el uso de la trombolisis local con catéter de ultrasonido en pacientes con TEP de riesgo intermedio fue segura y efectiva. Los resultados perioperatorios y a 30 días fueron comparables a los descritos en experiencias internacionales; sin embargo, aún se requieren de estudios con mayor número de pacientes para confirmar los beneficios de esta técnica en nuestro medio.


BACKGROUND: Pulmonary thromboembolism (PE) is a common cause of cardiovascular morbidity and mortality and local ultrasound-assisted thrombolysis (USAT) is a validated alternative treatment for intermediate-risk patients. There are no reports on the use of this technology in our country. METHODS: Prospective series of patients with acute, intermediate-risk PE treated percutaneously with therapeutic ultrasound catheter-assisted local thrombolysis (EKOSonic®). The efficacy and safety of the procedure were analyzed using clinical, hemodynamic, and radiological variables, as well as intra-hospital outcomes. The 30-day follow-up is also reported. RESULTS: between June 2019 and March 2020, a total of 4 patients with intermediate-risk PE were treated with this technique. The average PESI score was 79.5 + 6.1. Two out of 4 patients required the use of 2 catheters for bilateral therapy. The average reduction in systolic pressure of the pulmonary artery was 29% and 3 patients reversed the right ventricular dilation present at admission. The thrombotic burden was reduced by 20% according to the Computed Tomography Obstruction Index (CTOI). There were no intraoperative or in hospital complications associated with the intervention. CONCLUSION: In this initial series, the use of local thrombolysis with an ultrasound catheter in patients with intermediate-risk PE was safe and effective. The perioperative and 30-day outcomes were similar to those previously reported in international series. However, larger randomized trials are needed to confirm this potential benefit.

Abstract Objective To compare the effect of two therapeutic ultrasound protocols, with different times of exposure in the regeneration of critical bone defect. Methods Forty-five male rats were distributed among three experimental groups: therapeutic ultrasound group 5 minutes (TUG 5); therapeutic ultrasound group 10 minutes (TUG 10); and control group (CG). In all groups, a critical bone defect of 8.5 mm diameter was made in the calvaria region. The protocol was initiated on the 1st postoperative day in TUGs 5 and 10, with therapeutic ultrasound at the frequency of 1.0 MHz, pulsed mode, five times a week, at periods of 15, 30, and 60 days. Results Among the experimental groups, the highest volume of neoformation of osteoid matrix took place in the TUG 10 group followed by TUG 5, when compared with the CG group, in which the neoformation was restricted to the border region. The use of ultrasound promoted an increase in the thickness of the conjunctive matrix, proliferation of capillaries, alignment of the collagen fibers, reduction of edema and inflammatory process, being more significant in the 10-minutes time period. Conclusion Therapeutic ultrasound stimulated the repair of a critical bone defect, and the longer exposure time promoted greater osteogenic stimulation.


Resumo Objetivo Comparar o efeito de dois protocolos de ultrassom terapêutico com diferentes tempos de exposição para regeneração de defeito ósseo crítico. Métodos Foram utilizados 45 ratos, machos, distribuídos em três grupos: grupo ultrassom terapêutico 5 minutos (GUS 5); grupo ultrassom terapêutico 10 minutos (GUS 10); e grupo controle (GC). Em todos os grupos, confeccionou-se um defeito ósseo crítico, com 8,5 mm de diâmetro, na região da calvária. O protocolo foi iniciado no 1º dia do pós-operatório, no GUS 5 e no GUS 10, com ultrassom terapêutico na frequência de 1,0 MHz, modo pulsado, 5 vezes por semana, nos períodos de 15, 30, e 60 dias. Resultados Dentre os grupos experimentais, houve maior neoformação de matriz osteoide no GUS 10, seguido do GUS 5 quando comparados ao GC, no qual a neoformação foi restrita à região de borda. O uso do ultrassom promoveu aumento na espessura da matriz conjuntiva, proliferação de capilares, alinhamento das fibras colágenas, redução do edema e do processo inflamatório, tendo sido mais significativo no tempo de 10 minutos. Conclusão O ultrassom terapêutico estimulou o reparo do defeito ósseo crítico, e o maior tempo de exposição promoveu maior estímulo osteogênico.

Résumé

Introdução: As associações de Terapias Combinadas de Ultrassom e Eletroterapia (TCUE) no mercado nacional podem ser utilizadas através de diferentes equipamentos. Na prática clínica tem-se relatado que o uso das TCUE tem eficácia no tratamento da adiposidade localizada. Objetivo: Avaliar os efeitos da terapia combinada (TCUE) no tratamento da adiposidade localizada. Métodos: Trata-se de um estudo experimental, composto por uma amostra de 15 coelhos, de ambos os sexos, da espécie Oryctolagus Cuniculus, divididos aleatoriamente em três grupos: G1- grupo TCUE-M que utilizou efeitos do ultrassom com aplicação de correntes estereodinâmicas (n = 5); G2- grupo TCUE -H que utilizou uma combinação de ultrassom e corrente Aussie (n = 5) e G3- controle (n = 5) nenhuma terapia. Resultados: As análises dos resultados revelaram pequenas alterações na camada adiposa na maioria dos coelhos integrantes dos grupos tratados. Conclusão: A análise das lâminas histológicas apresentou irregularidades na membrana celular adiposa mostrando diferentes tamanhos de células. (AU)


Introduction: Associations of Combined Ultrasound Therapy and Electrotherapy (TCUE) can be used in the domestic market through different equipment. Some authors report that the use of TCUE has efficacy in the treatment of localized adiposity. Objective: To evaluate the effects of combined therapy (TCUE) in the treatment of localized adiposity. Methods: This is an experimental study consisting of a sample of 15 rabbits of both sexes of the species Oryctolagus Cuniculus, randomly divided into three groups: G1- TCUE-M group that used ultrasound effects with the application of stereodynamic currents (n = 5); G2-TCUE-H group that used a combination of ultrasound and current Aussie (n = 5) and G3-control (n = 5) no therapy. Results: Analyzes of the results revealed small alterations in the adipose layer in most of the rabbits in the treated groups. Conclusion: Analysis of the histological slides presented irregularities in the adipose cell membrane showing different cell sizes. (AU)

Abstract Objective: This clinical study sought to evaluate the effectiveness of passive ultrasonic activation (PUA) in eliminating microorganisms in primary endodontic infection (PEI) after instrumentation of root canals using microbiological culture and checkerboard DNA-DNA hybridization. Methodology: Twenty root canals with PEI and apical periodontitis were selected. The root canals were instrumented and then randomly divided into 2 groups, according to the irrigation method: PUA and conventional needle irrigation (CNI). Microbiological samples were collected before instrumentation (S1), after instrumentation (S2) and after irrigation with 17% EDTA (S3). The samples were subjected to anaerobic culture technique and checkerboard DNA-DNA hybridization analysis. Results: A statistically significant difference was found between CNI (23.56%) and PUA (98.37%) regarding the median percentage values for culturable bacteria reduction (p<0.05). In the initial samples, the most frequently detected species was S. constellatus (50%), and after root canal treatment was E. faecalis (50%). Conclusion: Both treatments significantly decreased the number of bacterial species compared with the initial sample. However, no statistical difference in the total microbial load between PUA and CNI groups was detected. The number of cultivable anaerobic bacteria reduced significantly using PUA, and the bacterial composition and number of bacterial species after using either CNI or PUA was similar.
